Specimen of caste (given by competent authority)

Copy of bank passbook or canceled check,

Proof of possession of two or more dairy cattle (Example / Certificate of Rural Milk Producers Association / Talati cum Minister).

Proof of ownership of land (Sample of Talati-cum Minister / Assessment Sheet / Certified copy of 7/12 and 8-A).

Beneficiary's consent / guarantee form.

Copy of Competent Officer's Disability Certificate (For Disabled Only)
