Who benefits?

In case of death or permanent disability of the beneficiary in any way other than suicide or natural death, the benefit of this scheme is available subject to the terms of the insurance policy.

How much benefit

Compensation under this scheme will be payable to the disabled persons covered under this scheme or his family within the maximum limit of sum assured of Rs.1,00,000 / -. The sum insured will be payable in the following circumstances keeping in view the terms of the general group accident insurance policy.

100% in case of death due to accident / permanent total disability,

100% in case of loss of two eyes or two limbs or arms / legs due to accident.

100% in case of loss of one eye and one limb due to accident.

50% of cases of loss of one eye or one limb due to accident,

Heirs

The following persons will be the successors of the beneficiaries under this insurance scheme.

Spouse - in their absence

Their baby son / daughter - in their absence

Their parents - in their absence

His grandchildren and great-grandchildren - in the aforesaid 1, 2, 3

W. Unmarried or widowed or abandoned sister living with the beneficiary and living with them,

Heirs declared under the Inheritance Act applicable to the beneficiaries except in the above case and in the disputed case.

In the above case if the heirs are minors then the sum insured will have to be paid to their natural guardian / legal guardian.

What evidence is required?

Not a moment in this year! The following procedure will be followed for the claim presented by the beneficiary's heir.

Benefits from the above in case of death. Heir or Permanent The beneficiary himself has to inform the nodal officer in writing within 90 days from the date of accident in the prescribed form under this scheme.

The following documents should be attached with the claim application.

Disability identity card.

F.I.R. 

Postmortem report of an authorized doctor.

Death certificate of the person issued by the birth-death registration officer.

Income certificate of annual income of a person with disability less than Rs. 6,00,000 / -.

Civil Surgeon of the concerned district in case of permanent disability of the local authorized medical officer Certificate.

Proof of age where required (school, college certificate)

Certificate of District Collector in case of impossibility of post-mortem report in exceptional cases.
