Who benefits?
40% or more should be disabled with 21 types of disability. There should be permanent residence in the state of Gujarat.
Income standard has been canceled by the Government.
What evidence is needed?
A certificate showing the percentage of the doctor to the specialist of that department.
2 (two) fresh stamped photos along with the application form.
Xerox copy of ration card.
Evidence of permanent residence in the State of Gujarat.
Blood group example.
Application forms can be obtained free of cost from the District Social Security Office.
The application form should be submitted to the District Social Security Office.
Certified copy of applicant's bank passbook.
Certified copy of the applicant's Aadhaar card.
Application should be made at District Social Security Officer, Bahumali Bhavan, Rajkot.
How much benefit
More than 80% blindness, low vision, low hemoglobin levels, cerebral palsy, mental illness, multiple sclerosis hardening of body tissues, impairment of developmental status of nervous system-neuro, deaf blindness
80% relief to a paralyzed person with hearing impairment, chronic neurological condition, tremor muscular stiffness, hemorrhagic-healed, impaired mobility
40% relief for a person with more than 100% intellectual disability, 50% relief for a mentally ill person, 50% relief for a person with a neurological developmental condition, 50% relief for a person with multiple disabilities, including deaf blindness.
Disposal of duplicate card in case of loss or cancellation of disability identity card
Divyang identity card will be available by applying for missing.
Duplicate card is issued by submitting the canceled identity card to the office.
